FOLLOW UP:
Please pay attention to your plan.  If I have requested a prostate exam, mammogram or labs please be compliant.  If you are asked to follow up after labs, or in 3 months or in 6 months, please be compliant. I usually send enough refills to last you until the next time you are to be seen.  If you are running out of refills, it usually means it's time for a follow up. PLEASE DO NOT get upset when I say no to refills if you have not been compliant. You are all adults (98%) so please take responsibility for yourself and your care.

To ALL Patients,

It is with a heavy heart that I am writing this letter to you today.  I opened The Hormone and Wellness Center with 30 patients in November 2008.  This has been one of the greatest challenges of my life and career.  I have had great support from Family, Friends and Patients.  As much as I have given you, I have received from working with you and that has been a great blessing.  Thank You, Thank You, Thank you.

Most of of you know that I have been traveling back and forth from Panama City to Houston for the past 3-4 years.  I have decided to make Houston my new home.  I have taken a job in Houston, providing the same services I have provided in Panama City.  I originally planned to continue seeing patients monthly in Panama City to keep the practice going.  My mind says I'm 30 something, but my body is suddenly reminding me that I am 56. It has become crystal clear that I will not be able to meet the needs of my patients in Panama City. 

That being said, I will be reducing my patient load in Panama City to 1/3 of my current patient load.  My priority is keep those patients who have been with me the longest.  Over the next two weeks, each of you will receive an email with a determination of your status with the practice.  I will remain available to all patient until the end of Feb 2023.  

Any patient that is being released from the practice will have an opportunity to be seen for a final visit and refills in Jan/Feb 2023.  Currently, there is another provider who is interested in seeing Hormone patients in the Panama City office.  If you would like to continue with said provider, I will provide her with your information.  You would become her patient and all services will be provided for you through her practice.  She understands that she may consult me on my previous patients if issues or problems arise. 

I will  continue to see the remaining patients in the Panama City office 4-5 times per year and telemed appointment will be available  3 days per month. These dates will be posted on the  "What Going On"  page of the website monthly. 

I will accept the following insurances for 2023:
    Medicare

    Tricare Plans ( Including Select, Tricare for life)
    United Healthcare (No UMR or Allsavers)
   
ALL other patients will pay cash based on the price list on our website.  (see "What Going On " page).  Payment methods will include: cash, check, HSA accounts, Zelle or Venmo payments.  Credit cards will be accepted WITH a 2.5% transaction fee.
